- Husband’s Date and Country of Birth
- Wife’s Maiden Name and Present Surname
- Wife’s Names

- Name of the Marriage Officer

Vault Copy
The vault copy is the most complete version of a marriage record. It is the original Marriage Register form used to officially record the marriage on the population register.
Typically completed by the marriage officer and signed by all parties, this document is often handwritten and preserved as an official record.
Key points about the vault copy:
- It is the original registration document
- Usually includes signatures of the couple and witnesses
- Earlier records (pre-1950s) may contain limited information
- Later records (from the late 1950s onward) often include significantly more detail, including identification numbers

Applications can be made at any branch of the Department of Home Affairs. Be sure to select the correct certificate type when submitting your application.
Applying for a Divorce Decree
It’s important to note that divorce records are not handled by Home Affairs. To obtain a divorce decree, you must apply at the court where the divorce was finalized. The Department of Home Affairs does not issue any divorce-related documents.
